Student Health Packet Requirement
Completion of the Student Health Packet is required for all new residential students living in campus housing. The Student Health Packet must be completed and sent to the Student Health Services Department prior to moving into campus housing. The date the Student Health Packet is due is listed on the Student Health Packet form. Failure to complete the Student Health Packet medical information and insurance requirements prior to moving into campus housing will jeopardize your housing placement and a financial hold will be placed on your student account. A financial hold will restrict the student from making any changes to the current and future course schedule.
Non-residential students are not required to complete the Student Health Packet.
